Título: PERI-IMPLANTITIS PRESENTING AS EXOPHYTIC PERI-IMPLANT SOFT TISSUE ENLARGEMENT: CASE REPORT

Resumo: Peri-implantitis is a biofilm-associated inflammatory condition characterized by peri-implant mucosal inflammation and progressive bone loss. A 57-year-old woman presented with a six-month history of progressive enlargement in the anterior mandibular peri-implant region. The implant had been placed approximately 10 years earlier without regular supportive maintenance. Clinical examination revealed a normochromic, sessile, firm, lobulated exophytic nodule on the mandibular alveolar ridge, with a smooth surface and focal ulceration, painful on palpation. Considering the persistent growth and uncertain clinical diagnosis, an excisional biopsy was performed. Intraoperatively, suppuration associated with the peri-implant tissues was observed. Histopathological examination showed inflamed granulation tissue compatible with peri-implant inflammatory disease. Based on the clinical, surgical, and histopathological findings, a diagnosis of peri-implantitis associated with reactive peri-implant soft tissue enlargement was established. The patient was referred for definitive peri-implant treatment and remains under clinical follow-up.
