Título: LYMPHANGIOMA OF THE UPPER LIP MIMICKING MUCOSAL VARICES: A DIAGNOSTIC CHALLENGE

Resumo: A 32-year-old female presented with a nodular lesion on the left upper lip, present since childhood and progressively enlarging with intermittent bleeding. She reported previous intervention without a conclusive diagnosis. Clinical examination revealed a vascular-appearing lesion with positive vitropression. Doppler ultrasound demonstrated a superficial low-flow image suggestive of a mucosal varix. Sclerotherapy with 5% Ethamolin was performed in two sessions, resulting in significant volume reduction. Due to the persistence of a small hardened residual area and aesthetic concern, an excisional biopsy was performed for diagnostic and therapeutic purposes. Histopathological analysis revealed dilated vascular spaces lined by flattened endothelium and containing proteinaceous material, consistent with lymphangioma. The upper lip is an uncommon site for this lesion, which more frequently affects the tongue. This case highlights limitations of imaging in differentiating vascular from lymphatic lesions and underscores the importance of histopathological analysis for definitive diagnosis. Postoperative follow-up demonstrated adequate healing without recurrence.
