Resumo: Paracoccidioidomycosis is a systemic fungal infection classically associated with rural exposure, making its occurrence in urban patients a diagnostic challenge. This report describes two cases of oral paracoccidioidomycosis in patients from Campinas, São Paulo, Brazil. A 19-year-old female presented with a two-month history of painful gingival inflammation, showing a moriform appearance. An incisional biopsy confirmed the diagnosis, and the patient was referred to an infectious disease specialist. A 52-year-old male, a private school teacher, presented with a five-month history of gingival inflammation. Histopathological analysis following biopsy established the same diagnosis. Both patients were treated with sulfamethoxazole-trimethoprim and showed complete clinical remission. They remain under follow-up without evidence of recurrence. These cases emphasize the importance of including paracoccidioidomycosis in the differential diagnosis of chronic gingival lesions, even in urban populations, and reinforce the role of biopsy for early diagnosis.
