Título: PREDICTORS OF MALIGNANT TRANSFORMATION IN ORAL POTENTIALLY MALIGNANT DISORDERS OVER 25 YEARS

Resumo: This study aimed to provide a comprehensive assessment of patient profiles with oral potentially malignant disorders, focusing on factors that may influence lesion severity and prognostic outcomes. This study was approved by the ethics committee. A retrospective analysis of histopathological records from 2000 to 2025 was performed. A total of 918 lesions clinically diagnosed as leukoplakia, erythroplakia, or leukoerythroplakia and histopathologically confirmed as epithelial dysplasia were included. Data on age, sex, race, smoking habits, clinical characteristics, anatomical site, lesion size, histopathological diagnosis, and follow-up data were collected. Cases with available follow-up were reclassified histopathologically according to the WHO 2022 and binary grading systems. Statistical analyses were performed. Follow-up data were available for 74 patients. Favorable progression was observed in 41 (55.4%) cases, 15 (20.3%) cases showed recurrence, and 18 (24.3%) malignant transformation (MT). Patients with MT were mostly female (12 cases, 66.7%) and smokers (8 cases, 44.4%), with mean age of 60.8 years. 10 (55.5%) lesions were classified as leukoplakia, 14 (77.8%) single lesions, 14 (77.8%) located in high-risk sites, 8 (44.4%) diagnosed histopathologically as moderate dysplasia. The mean time to malignant transformation was 52 months. Standardized follow-up strategies may contribute to earlier detection of progression and improved patient management.
