Título: Fractal Dimension of Mandibular Bone in Cardiovascular Disease: a cross-sectional study using panoramic radiograph

Resumo: Aim: To assess mandibular bone microarchitecture through fractal dimension (FD) analysis in individuals with cardiovascular disease (CVD) and to investigate its association with clinical variables. Study Design: A matched comparative cross-sectional study was conducted with adults diagnosed with CVD and healthy controls, paired by age and sex. Panoramic radiographs (PR) obtained under standardized conditions were analyzed using ImageJ with the FracLac plugin. FD was calculated in the mandibular angle, ramus, and mental regions. Sociodemographic and clinical data were collected, and statistical analyses included group comparisons and logistic regression. Results: A total of 142 individuals were included (71 CVD; 71 controls; median age 64 years). The CVD group exhibited significantly lower FD values in all regions analyzed, particularly in the mandibular ramus (p < 0.001). Reduced FD values were also observed in patients with coronary artery disease and among diuretic users in the ramus region (p < 0.05). Other clinical variables were not significantly associated with FD in adjusted models. Conclusion: FD analysis from PR revealed reduced mandibular bone complexity in patients with CVD, suggesting potential alterations in bone mineral density. Clinical factors may influence these findings, and further studies with standardized protocols are required to validate its diagnostic applicability.
