Resumo: A 19-year-old female patient sought medical attention due to rapidly growing lip pigmentation that had been developing for two months. Her medical history included asthma and use of oral contraceptives, with no significant sun exposure or smoking. Intraorally, a brownish spot with indistinct borders and variable coloration, measuring 2.5 cm, was observed, associated with mild burning. Hypotheses included melanoma, Addisons disease, and melanoacanthoma. Incisional biopsy revealed numerous dendritic melanocytes, acanthosis, and chronic inflammatory infiltrate, confirming the diagnosis of melanoacanthoma. After the biopsy, there was partial regression, but the lesion subsequently recurred. A new partial biopsy was performed, with no change in the clinical picture. The anamnesis revealed the use of volumizing lip cosmetics; after discontinuation, the lesion partially involuted, leading to the decision to perform complete excision. This case highlights the importance of thorough clinical evaluation and anamnesis in the diagnosis of oral pigmented lesions.
