Resumo: Microsecretory adenocarcinoma is a rare, recently described low-grade salivary gland carcinoma of minor salivary glands. A 42-year-old female presented with a 2 cm asymptomatic nodule on the left soft palate with slow growth. Complete excision was performed based on a clinical impression of a benign lesion. Histopathological examination showed a well-circumscribed, non-encapsulated salivary gland neoplasm composed of microcystic structures lined by flattened epithelial cells with eosinophilic to clear cytoplasm and PAS-diastase-positive secretory material, with focal cribriform areas in a fibromyxoid stroma. The overlying epithelium exhibited pseudoepitheliomatous hyperplasia, and focal tumor-associated lymphoid proliferation was present. Tumor cells were positive for CK7, S100, SOX10 and p63, and negative for mammaglobin, calponin, and p40. The patient remains disease-free after 6 months. This rare case highlights the importance of recognizing this entity and its mimics; diagnosis was established based on morphology and immunohistochemistry, as molecular testing for MEF2C::SS18 was unavailable.
