Título: CALCIUM HYDROXIDE EXTRUSION INTO THE MANDIBULAR CANAL: CLINICAL, RADIOGRAPHIC FINDINGS, AND MANAGEMENT

Resumo: A 32-year-old female patient underwent endodontic treatment of tooth 47 using an intracanal medicament based on calcium hydroxide, which resulted in apical extrusion of the material into the mandibular canal, with subsequent dissemination to adjacent soft tissues and extension to the palatal region. The clinical course was characterized by severe pain, facial edema, paresthesia in the distribution of the inferior alveolar nerve, submandibular lymphadenopathy, and necrosis of the palatal mucosa. Imaging examinations revealed radiopaque material extruded beyond the apex, in close proximity to the mandibular canal, associated with periapical bone rarefaction and C-shaped root canal anatomy. The diagnosis of chemical injury secondary to calcium hydroxide extrusion was established. Management included extraction of tooth 47, surgical debridement, irrigation with saline solution, topical application of benzydamine hydrochloride, and low-level laser therapy. After three months, complete tissue repair was observed; however, partial paresthesia persisted and remained evident after two years of follow-up.
