Resumo: Midface defects secondary to tumor resection pose significant functional and aesthetic challenges. CASE REPORT: A 38-year-old male patient was diagnosed with squamous cell carcinoma in the left maxilla. Tumor resection was performed, followed by reconstruction with a microvascularized fibular flap (MRF). The proposed technique consisted of using the MRF in two segments: the first was used to reconstruct the anterior region of the maxilla, and the second to reconstruct the left alveolar process. This was fixed to the remaining stump of the zygomatic arch with a 2.0 titanium miniplate and screws, with osteotomy performed in the fibula for adaptation ("gap"). Vascular anastomosis was performed with the facial artery and vein. The coronoid process was removed due to its intimate contact with the tumor. The prosthetic space was preserved. CONCLUSION: This surgical reconstruction technique proved satisfactory in terms of both function and aesthetics. Keywords: Fibula; Squamous Cell Carcinoma; Reconstruction; Maxilla
