Resumo: Burkitts Lymphoma is a highly aggressive lymphoproliferative neoplasm often associated with extranodal involvement, including the oral cavity. This report describes two cases with initial oral presentation in young male patients, both showing rapid clinical progression. In the first case, marked axillary lymphadenopathy was observed along with exophytic lesions in the mandibular gingiva and retromolar trigone, extending into the masticatory space. In the second case, diagnosis occurred during a consultation at a cancer prevention service after identification of a progressive gingival and palatal lesion initially treated as periodontal inflammation. Rapid growth and severe pain led to biopsy. Imaging studies, including computed tomography and PET-CT, revealed expansive lesions in the jaw and systemic dissemination with increased glycolytic activity. Diagnosis was confirmed by histopathology and immunohistochemistry, demonstrating a "starry sky" pattern, positivity for CD20, CD10, BCL6, C-MYC, and a high proliferative index (Ki-67), reinforcing the disease's aggressiveness and need for early detection.
