Resumo: Objective: To quantify depth of invasion (DOI) and tumor thickness (TT) in oral squamous cell carcinoma (OSCC) arising in clinically homogeneous leukoplakia and define the minimum biopsy depth required to reliably detect early invasion.
Study design: Retrospective cross-sectional study of histopathological records from a single oral pathology service (1997-2025). Cases clinically diagnosed as homogeneous leukoplakia, presenting as uniform white plaques, were included when OSCC was histologically confirmed and surgical specimens allowed precise DOI and TT measurement according to College of American Pathologists criteria. Additionally, high-risk histological features were assessed.
Results: Eight cases were eligible. Mean DOI was 0.62±0.34 mm (range: 0.19-1.03 mm), and mean TT was 0.92±0.52 mm (range: 0.60-2.05 mm). No tumors exhibited perineural or lymphovascular invasion. All cases showed low-grade patterns of invasion (grades 1-3). In all specimens, invasive fronts were consistently superficial, remaining within the first millimeters of tissue.
Conclusion: Early invasive OSCC arising in homogeneous leukoplakia is characteristically shallow. A biopsy depth of approximately 3 mm is likely sufficient to capture invasion and avoid underdiagnosis ensuring a deep margin clearance of at least 1 mm. These findings provide a clinically actionable benchmark but require validation in larger prospective cohorts.
