Título: Non-healing chronic traumatic ulcers exhibit immunophenotypic features of oral carcinogenesis

Resumo: Objective: Non-healing chronic traumatic ulcers (NHCTU) have been proposed as lesions with malignant potential, particularly on the lateral tongue. This study investigated immunophenotypic profiles consistent with early oral carcinogenesis in NHCTU.
Study design: Twenty patients were included in each of the following 5 groups: NHCTU, reactive fibrous hyperplasia (RFH), leukoplakia, de novo oral squamous cell carcinoma (OSCC), and sequential OSCC. Immunohistochemistry for p53, p63, Ki-67, cyclin D1, and Bcl-2 was analyzed using quantitative digital image analysis (ImageJ/IHC Toolbox; % positive epithelial area) and qualitative epithelial distribution patterns (basal/parabasal/suprabasal/full thickness).
Results: Quantitative analysis revealed significant differences for p53 (p=0.0162), Ki-67 (p=0.0115), and p63 (p<0.0001), while cyclin D1 and Bcl-2 were not significant (p=0.2587 and p=0.10). NHCTU showed intermediate p53 values (higher than RFH, like leukoplakia, lower than OSCC) and the highest mean Ki-67. Qualitatively, NHCTU frequently showed parabasal/suprabasal extension of p53, p63, and Ki-67. Cyclin D1 was mainly parabasal, and Bcl-2 was predominantly negative.
Conclusions: A subset of NHCTU exhibits increased proliferative activity and suprabasal extension of biomarkers of early carcinogenesis. These findings suggest that some NHCTUs may have changes that predispose them to carcinogenesis, and highlight the need for proteomic or genomic studies to clarify possible pathological pathways.
