Título: EVALUATION OF CLINICAL DIAGNOSTIC ACCURACY IN ORAL POTENTIALLY MALIGNANT DISORDERS AND ORAL CANCER

Resumo: Objective: To evaluate the diagnostic accuracy of clinical hypotheses in identifying oral cancer (OC) among oral potentially malignant disorders (OPMDs) and benign lesions in a university-based stomatology service. Study design: A retrospective diagnostic accuracy study based on the review of clinical records of patients between 2021 and 2025, following STARD recommendations. The initial clinical hypothesis was considered the index test, and the histopathological diagnosis was used as the reference standard. Sensitivity, specificity, predictive values, overall accuracy, and Cohen's Kappa coefficient were calculated ts. Results: Sixty cases comprised the final analysis. Among the analyzed cases, OPMDs predominated (58.3%), followed by OC (23.3%) and benign lesions (18.3%), with leukoplakia being the most frequent OPMD and squamous cell carcinoma the main malignant neoplasm diagnosed. Plaque was the most prevalent primary lesion type. The buccal mucosa was the most affected site, followed by the lips and tongue. Clinicopathological concordance was observed in 73.3% of cases, with moderate agreement. In the dichotomized analysis, clinical diagnosis showed 78.6% sensitivity and 84.8% specificity, positive predictive value of 61.1% and a negative predictive value of 92.9%. Clinical suspicion of malignancy was significantly associated with histopathological confirmation. Conclusion: Histopathological confirmation remains essential for definitive diagnosis in OPMDs and OC.
