Resumo: A 70-year-old female presented to a private practice office with a lesion beneath a full-arch implant-supported fixed prosthesis. Clinical examination revealed a large, mucosa-colored nodule on the soft tissue, with a prosthesis-molded contour, ulceration, and adjacent whitish areas. The patient's medical history included smoking and alcohol use, and the lesion had been present for three months. Differential diagnoses included inflammatory fibrous hyperplasia and squamous cell carcinoma.

An incisional biopsy showed connective tissue with skeletal muscle fibers and multiple round, optically clear spaces consistent with PMMA microspheres, surrounded by a foreign body-type granulomatous reaction with CD68-positive cells and fibrosis.

Findings supported a diagnosis of foreign body reaction to exogenous material, likely PMMA filler. Although initially denied, the patient later reported prior lip filling. This case underscores how filler-related reactions may closely mimic common reactive lesions and even malignancies, reinforcing the critical role of thorough history-taking and clinicopathologic correlation in avoiding misdiagnosis.
