Resumo: This is a report case of a 67-year-old female patient, leukoderma, hypertensive, diabetic, presenting with well-defined, verrucous-appearing leukoplakic plaques that could not be removed by scraping. The smaller lesions were located on the gingival margin and the tongue, while a larger lesion, measuring approximately 8 cm, extended from the right buccal mucosa to the anterior mandibular gingival margin. Based on the diagnostic hypothesis of proliferative verrucous leukoplakia (PVL), a toluidine blue test (TBT) was performed, yielding positive results on the tongue and buccal mucosa. These areas were selected for incisional biopsy. Histopathological analysis revealed acanthosis, hyperkeratosis, and severe epithelial dysplasia, findings consistent with PVL. After the patient was informed about the condition and therapeutic options, a decision was made to perform monthly follow-up with TBT, with biopsy indicated only in case of positive findings. The patient has been followed for 4 months, with no indication for repeat biopsy to date.
