Título: EPIDEMIOLOGICAL ANALYSIS OF ORAL SQUAMOUS CELL CARCINOMA AND PRECURSOR LESIONS: A RETROSPECTIVE STUDY IN ONTARIO, CANADA

Resumo: Objective: To identify temporal trends of oral squamous cell carcinoma (OSCC) and oral epithelial dysplasia (OED) from 2015-2024, examining demographic, clinical and histologic patterns, including changes associated with the COVID-19 pandemic.
Study Design: A retrospective review of two oral biopsy service databases in Ontario, Canada, was conducted. Demographic, clinical and histologic features were evaluated for trends in OSCC and OED diagnoses over a 10-year period.
Results: Among 70,495 cases, biopsy submissions increased by 25% from 2015 to 2024, outpacing population growth. A notable decline occurred at the onset of the COVID-19 pandemic, followed by a significant rebound. OSCC diagnoses rose from 94 to 151 cases (28.4%, p<0.025), while OED increased from 347 to 656 cases (51.1%). OSCC most frequently involved the ventrolateral tongue (34.2%) and gingiva (33.1%), with a decreasing trend in the palate and floor of mouth. Demographic data indicate an increase in dysplasia cases among older, non-smoking patients.
Conclusion: Rising diagnoses of OSCC and OED, paired with increased biopsy submissions, suggest an evolving landscape in oral cancer detection. The post-COVID-19 surge in OSCC underscores the need for continued surveillance. The expanding role of general dentists in referrals highlights an important opportunity for early detection and improved patient outcomes.
