Título: MANDIBULAR OSTEORADIONECROSIS WITH PATHOLOGICAL FRACTURE: A 20-YEAR CLINICAL FOLLOW-UP

Resumo: A patient previously treated with radiotherapy for lower lip carcinoma presented with initial complaint of pain and extraoral fistula, later developing mandibular osteoradionecrosis in the right posterior region after molar extraction, progressing to pathological fracture. Diagnosis was established based on history of head and neck irradiation, clinical evidence of persistent bone exposure, and imaging findings consistent with necrotic bone and mandibular discontinuity. Since 2006, the patient has been followed in an Oral Medicine service under a conservative management approach. Over a 20-year follow-up, the condition remained clinically controlled despite persistence of radiographic bone alteration without progression. In 2025, tooth #38 presented severe mobility and underwent spontaneous avulsion. Alveolar curettage was performed, followed by five sessions of antimicrobial photodynamic therapy (12 J per session), resulting in clinical improvement and satisfactory mucosal epithelialization. This case highlights the importance of long-term follow-up and conservative management in irradiated patients.
