Resumo: A 16-year-old male presented with submandibular swelling. Clinical examination revealed cystic lesions associated with the floor of the mouth. Intraoral assessment demonstrated bilateral nodular enlargements adjacent to the lingual frenulum. The clinical differential diagnosis included oral lymphoepithelial cyst, sialolithiasis, ranula, and salivary duct cyst. Bilateral excisional biopsies were performed in the floor of the mouth near the lingual frenulum, and specimens were submitted for histopathological evaluation. Microscopic analysis confirmed salivary duct cysts on both the right and left sides. The postoperative course was uneventful, with satisfactory healing and no functional complications. The patient was discharged from Oral Medicine care and remains under periodic observation without evidence of recurrence. Bilateral salivary duct cysts are rare, particularly in adolescents, making early diagnosis and surgical management essential to prevent recurrence and misdiagnosis.
