Resumo: Oral metastases are rare and may resemble benign or reactive lesions, making diagnostic challenges. A 60-year-old male presented with a rapidly enlarging gingival mass in the mandibular premolar-molar region that doubled in size within three weeks. The lesion measured 3 × 3 cm, with erythematous areas, firm consistency, and focal ulceration. The patient had a history of pulmonary adenocarcinoma with cervical metastasis and was under chemotherapy. Incisional biopsy revealed a malignant epithelial neoplasm with marked pleomorphism, atypical mitoses, necrosis, and vascular invasion. Immunohistochemistry was positive for pan-cytokeratin and TTF-1, and negative for p63, CD45, CD30, and S100. Metastatic pulmonary adenocarcinoma was diagnosed. The patient was referred for oncologic management and remains under palliative care. This case highlights the importance of considering metastasis in rapidly growing gingival lesions.
