Título: Silent Oral Infection in Rheumatoid Arthritis

Resumo: We present two clinical cases of female patients, aged 64 and 66, diagnosed with rheumatoid arthritis and on continuous methotrexate therapy, who were evaluated during routine dental visits. Neither patient presented specific dental complaints, however, clinical examination revealed diffuse erythematous areas on the hard palate and the superior anterior alveolar ridge, consistent with erythematous candidiasis. Their DAS-28 scores were 3.63 and 3.77, respectively. Systemic immunomodulation associated with the use of disease-modifying antirheumatic drugs may attenuate the local inflammatory response, favoring opportunistic infections with subtle and insidious clinical presentation. The patients received oral hygiene guidance and topical antifungal treatment, with clinical follow-up, resulting in a reduction in DAS-28 to 2.59 and 1.78, and complete control of the infection. The findings highlight the need for qualified clinical surveillance for erythematous Candida infections, which may represent a potential source of complications in patients with rheumatoid arthritis undergoing immunosuppression.
