Título: ORAL METASTASIS OF BREAST CARCINOMA PRESENTING AS AN ODONTOGENIC-LIKE LESION IN THE MANDIBLE: A DIAGNOSTIC PITFALL

Resumo: A 55-year-old woman presented with a 10-month history of persistent pain in the left posterior mandible, evolving to progressive swelling. Clinical examination revealed tooth mobility (33-36) and a firm, normochromic submucosal mass measuring approximately 6 cm, accompanied by submandibular enlargement. Computed tomography disclosed an expansive osteolytic lesion with a multicystic appearance, initially suggestive of ameloblastoma. Following incisional biopsy, histopathological analysis demonstrated an infiltrative epithelial neoplasm with multinodular architecture, mild-to-moderate nuclear pleomorphism, and frequent atypical mitoses. Immunohistochemistry showed diffuse positivity for AE1/AE3, CK7, GATA3, estrogen and progesterone receptors, mammaglobin, GCDFP-15, and Ki-67 (~20%), supporting the diagnosis of metastatic breast carcinoma. The patient was referred for oncologic management and remains under follow-up. This case underscores the deceptive clinical presentation of metastatic disease in the jaws and highlights the critical role of integrated histopathological and immunophenotypic evaluation.
