Resumo: A 15-year-old male patient presented with frontal bossing, hypertelorism, coarse facial features, and palmar pits. Radiographic evaluation revealed multiple lesions affecting the maxilla and mandible, associated with calcification of the falx cerebri, supporting the diagnosis of Gorlin-Goltz syndrome based on established clinical criteria. Considering the patients age and the multifocal presentation, a conservative and staged approach was adopted, consisting of marsupialization followed by enucleation and curettage. Histopathological analysis confirmed the lesions as odontogenic keratocysts. Longitudinal clinical and radiographic follow-up over a 3-year period demonstrated progressive bone regeneration, preservation of anatomical structures, and good treatment adherence. During follow-up, a hypodense area in the region of teeth 35 and 36 was identified, requiring further monitoring. This case highlights the importance of early diagnosis, individualized management, and continuous long-term follow-up in controlling lesion recurrence and ensuring favorable outcomes in patients with Gorlin-Goltz syndrome.
