Resumo: A 61-year-old male patient was referred by an otorhinolaryngologist and a dentist due to an erythroleukoplakic lesion in the retromolar trigone, extending to the soft palate and tonsillar pillar. Clinical examination revealed a lesion with high-risk features for malignant transformation, and excisional biopsy was performed. Histopathological analysis demonstrated hyperkeratosis associated with moderate to severe epithelial dysplasia. Based on these findings, the patient was promptly referred to a head and neck oncology team. Surgical resection involving the soft palate and right margin was performed. Final histopathological diagnosis confirmed high-grade epithelial dysplasia/carcinoma in situ with ulceration. Postoperative follow-up was indicated. During follow-up after oropharyngeal surgery, the area showed a whitish, scar-like appearance, without clinical signs of recurrence or malignant transformation. This case highlights the importance of early diagnosis, timely surgical intervention, and multidisciplinary management in preventing progression to invasive carcinoma and improving patient outcomes.
