Título: Glandular Odontogenic Cyst of the Mandible with Differential Diagnosis for Renal Cell Carcinoma Metastasis: A Case Repor

Resumo: A 46-year-old female presented a painful swelling in the right mandible for 2-3 months and a history of renal cell carcinoma. Clinical examination showed premolar swelling, cortical plates enlargement, normal overlying mucosa, tooth mobility, and loss of vitality. Imaging revealed a multilocular intraosseous radiolucency with "soap bubble/honeycomb" appearance and cortical involvement. Differential diagnoses included ameloblastoma, odontogenic myxoma, and metastatic renal cell carcinoma. Biopsy revealed a cystic cavity lined by epithelium of variable thickness with cuboidal to polygonal cells showing eosinophilic or clear cytoplasm, pseudoductal structures, epithelial crypts, mucous cells, hobnail cells, and focal whorl-like arrangements. Although clear cells and cystic architecture raised concern for metastatic cystic clear cell renal carcinoma, the presence of mucous cells, glandular features and immunohistochemical profile (CK7 and CK19) supported the diagnosis of glandular odontogenic cyst. Surgical excision with involvement of the basal margin was performed, and long-term follow-up was established due to recurrence risk.
