Resumo: This case series describes two patients with chronic areca nut use presenting with lesions demonstrating salivary ductal alterations on histopathologic examination. The first patient, a 76-year-old female with a 40-year history of use, presented with a painful indurated ulcer on the floor of the mouth of one month's duration. The second, an 86-year-old female, presented with a painless right buccal mass of six months' duration. Histologically, the first showed ulcerated epithelium and multiple salivary ducts within the lamina propria exhibiting proliferative changes, including luminal snouting, papillary projections, ductlike microcystic structures, and mucous cells. The second demonstrated a fibrous lesion with proliferative ductal elements with mild cytologic atypia. Immunohistochemical studies and follow-up were available for the first, supporting benign reactive changes, with near-complete resolution after incision and no recurrence. These findings expand the histopathologic spectrum of areca nut-associated changes and highlight a potential diagnostic pitfall, as they may mimic malignancy.
