Resumo: Syphilis, an STI caused by Treponema pallidum, progresses through three stages: primary (a single ulcerative lesion), secondary (hematogenous dissemination with multiple mucocutaneous lesions), and tertiary (destructive systemic processes). A notable case involved a 15-year-old female presenting persistent pain and dysphagia for two months. Despite being in good general health, intraoral examination revealed multiple erythematous, rounded lesions, some confluent, with well-defined borders and smooth to slightly ulcerated surfaces involving the posterior dorsum of the tongue, palate, buccal mucosa, and labial mucosa. Serological testing for hepatitis B, C and HIV was negative, while testing for syphilis was positive, confirming the secondary stage. The patient was promptly referred to an infectious disease specialist. This case underscores the importance of clinical and laboratory correlation, ethical management, and appropriate referral. It also highlights the rising incidence of syphilis among adolescents and the crucial role of the dentist in early diagnosis and prevention of transmission.
