Resumo: An adult female patient presented with progressive swelling in the posterior mandible associated with facial asymmetry. Clinical examination revealed mandibular augmentation, with expansion of the mandibular body and soft tissue. Computed tomography demonstrated a well-defined intraosseous lesion of mixed density, with marked cortical expansion, suggesting a central fibro-osseous neoplasm. Given the expansive behavior and extent of the lesion, segmental mandibular resection was performed, followed by immediate reconstruction with a titanium plate, and a second surgery to reconstruct with bone graft. Histopathological analysis revealed cellular fibrous connective tissue associated with foci of mineralized material, confirming the diagnosis of ossifying fibroma. This case highlights an uncommon presentation of a lesion with aggressive features and underscores the importance of comprehensive clinicoradiographic and histopathological correlation for accurate diagnosis and appropriate treatment planning.
