Título: SCURVY AS A DIFFERENTIAL DIAGNOSIS OF HEMATOLOGIC MALIGNANCIES IN PEDIATRIC PATIENT: A CASE REPORT

Resumo: A 4-year-old boy with a previous diagnosis of autism spectrum disorder, level 3, was referred to an oncology center for diagnostic investigation due to complaints of an "oral mass" and gingival bleeding. The patient also presented progressive food refusal, weight loss, pallor, asthenia, severe pain in the lower limbs, and fever. Intraoral examination revealed hypertrophic and erythematous/purplish marginal gingiva, with no clinical signs of odontogenic infection. Acute leukemia and Burkitt lymphoma were initially considered among the diagnostic hypotheses. Hematological investigation revealed severe anemia without thrombocytopenia. Bone marrow aspiration ruled out neoplastic infiltration. Considering the markedly restrictive diet and clinical findings, scurvy was suspected, and a very low serum vitamin C level (0.06mg/dL) was detected. Vitamin C supplementation (100mg/day for 7 days) led to complete resolution of gingival manifestations. Normalization of hemoglobin levels was observed after six weeks. Dentists should be aware that systemic conditions may influence oral manifestations of diseases.
