Título: ORAL MANIFESTATION OF MULTIPLE MYELOMA PRESENTING AS MANDIBULAR OSTEOMYELITIS: A CASE REPORT

Resumo: A 51-year-old man with kappa light-chain multiple myeloma and vertebral metastasis, undergoing treatment with belantamab and dexamethasone, in addition to sulfamethoxazole/trimethoprim, acyclovir, and allopurinol, was referred to Oral Medicine due to suspected mandibular osteomyelitis. Clinical examination revealed pain and bone exposure in the left mandibular body region. An incisional biopsy was performed in the affected area, associated with extraction of tooth 35. Histopathological analysis demonstrated a hematolymphoid neoplasm. Immunohistochemical evaluation showed a Ki-67 proliferative index of approximately 50% in hotspot areas. Kappa, lambda, CD3, and CD20 markers were inconclusive. Findings were consistent with oral involvement of hematologic malignancy. The patient was referred back to the oncology team for systemic reassessment and therapeutic planning. Close clinical and imaging follow-up was established.
