Resumo: Objective: To characterize the epidemiological and histopathological profile of lesions diagnosed in a hospital-based anatomical pathology laboratory. Study design: Retrospective cross-sectional study. Medical records and histopathological reports issued between January 2020 and September 2025 were analyzed using specific anatomical descriptors of the oromaxillofacial region. Statistical significance was set at p < 0.05. Results: A total of 87 histopathological reports were identified, with a predominance of lesions in women (57.5%). The mean age was 53 years (SD = 20), and the 60-69-year age group was the most prevalent (23.3%). The most commonly affected anatomical sites were the lower lip (35.7%) and upper lip (29.8%). The main clinical diagnostic hypotheses were neoplasia (10.3%), squamous cell carcinoma (9.2%), Sjögren's disease (6.9%), and fibrous hyperplasia (5.7%). Histopathological analysis revealed a predominance of squamous cell carcinoma (21.8%) and basal cell carcinoma (10.3%), followed by actinic cheilitis (5.7%) and fibrous hyperplasia (5.7%). Agreement between clinical and histopathological diagnoses was fair (Kappa = 0.35). Conclusion: The high prevalence of malignant and potentially malignant lesions highlights the importance of epidemiological surveillance in pathology services to improve early diagnosis and ensure appropriate management of oral lesions in the hospital setting.
