Resumo: A 40-year-old male patient presented to clinic with an asymptomatic volumetric increase in the posterior left mandible, approximately 8 cm in length and with a duration of two years. Intraoral examination revealed a well-defined, purplish lesion and absence of tooth 36. The patient brought with him the tomographic examinations performed for follow-up two years ago, in which a hypodense, unilocular area could be observed containing a hyperdense area in its central proportion, with thinning of the lingual cortical bone. These areas were bilateral in the mandible. The hypothesis was secondary infected cementum dysplasia leading to osteomyelitis, but the biopsy revealed the presence of two distinct lesions, with diagnoses of central giant cell lesion (CGL) and florid cemento-osseus dysplasia. The association of these two lesions has been found in few cases reported in the literature. The patient underwent surgical treatment with removal of the lesion and is currently under follow-up.
