Título: ATYPICAL GRANULAR CELL TUMOR OF THE TONGUE: A RARE CASE REPORT

Resumo: Granular cell tumor (GCT) is a rare neoplasm of Schwann cell origin, accounting for 0.5% of all soft tissue tumors. While typically benign, approximately 2% of cases undergo malignant transformation. We report a case of atypical GCT on the tongue dorsum of a 38-year-old woman. Physical examination revealed a firm, pedunculated nodule with a small area of ulceration, measuring approximately 0.5 cm. Based on a clinical impression of fibrous hyperplasia, an excisional biopsy was performed. Histopathological examination revealed a neoplasm composed of numerous spindle-shaped cells and scarce round cells with eosinophilic granular cytoplasm. The overlying epithelium showed pseudoepitheliomatous hyperplasia. Immunohistochemically, tumor cells were positive for S100 and CD68 but negative for AE1/AE3. A definitive diagnosis of atypical GCT was established. The patient remains disease-free, with no signs of recurrence at the 18-month follow-up. This case underscores the importance of identifying atypical or malignant presentations to ensure early diagnosis and intervention.
