Título: RITUXIMAB IN THE TREATMENT OF PEMPHIGUS VULGARIS: A SYSTEMATIC REVIEW AND META-ANALYSIS

Resumo: Objective:
Evidence on therapeutic strategies for Pemphigus vulgaris, focusing on pain scores, and adverse effects.
Study design:
A systematic review and meta analysis was conducted following a search in PubMed, Embase, Scopus, Cochrane Library, Web of Science, and LILACS. With protocol register: Open Science Framework. Duplicate records were removed using Rayyan. Risk of bias was assessed with the Joanna Briggs Institute Critical Appraisal Tools. Meta analyses were performed using R (version 3.6.3) with the package META R, and certainty of evidence was evaluated using the GRADE approach.
Results:
Ten studies included. Most investigations evaluated Rituximab as a therapeutic option, with heterogeneous reporting of lesion location, previous treatments, comorbidities, and outcomes. Early initiation of rituximab was associated with higher disease-free rates, reduced need for adjuvant therapy, and lower recurrence. Adverse events were mainly infusion related. Meta analysis demonstrated a statistically significant reduction in adverse effects favoring rituximab (OR = 6.64; 95% CI: 1.52-29.09; p = 0.0119), despite high heterogeneity (I˛ = 98.6%). Certainty of evidence was rated as moderate.
Conclusion:
Systemic corticosteroids remain essential for rapid disease control, while rituximab demonstrates superior efficacy and safety, supporting individualized treatment strategies to optimize remission and reduce adverse events.
