Título: MISSED ADJUVANT THERAPY IN HIGH-RISK CUTANEOUS SQUAMOUS CELL CARCINOMA

Resumo: Objective: Identify factors associated with missed and incomplete adjuvant therapy after regional metastasis in the context of head and neck cutaneous squamous cell carcinoma (HNCSCC).
Methods: Patients with resected HNCSCC from 2011 to 2021 and known regional disease were retrospectively included.
Results: In total, 42 patients were included. 21 (50%) completed adjuvant radiation (mean dose 51.6 Gy), 15 (35.7%) missed and 6 (14.2%) received incomplete treatment (mean dose 29.3 Gy). The most documented reason for not initiating adjuvant therapy was loss of clinical performance, followed by patient refusal. The most common reason for adjuvant therapy suspension was intolerance. Older patients were less likely to complete adjuvant therapy (Mean age 80.15 vs 70.83, p=0.042), with a tendency of higher risk of missing adjuvant treatment in neck metastasis versus parotid only disease (parotid only in 6 (28.5%) vs 12 (57.1%) cases, p=0.061). Receipt of adjuvant radiation (General response in 2 (9.5%) vs 15 (71.4%) cases, p<0.001) were associated with higher probability of response to treatment.
Conclusion: A significant proportion of CSCC do not receive appropriate adjuvant therapy. Older patients may be at higher risk of missing adjuvant therapy, and missed adjuvant therapy is associated with poor general response to treatment.
