Resumo: Odontogenic keratocyst (OKC) is a developmental cyst characterized by aggressive behavior and a high recurrence rate, most commonly affecting the posterior mandible. Maxillary involvement with extension into the maxillary sinus is uncommon. A 16-year-old female patient was referred for evaluation of an intraosseous lesion detected on panoramic radiography after reporting tooth displacement in the region of the right maxillary first and second molars. Imaging revealed a radiolucent lesion occupying the right maxillary sinus and enveloping the crown of the right maxillary second molar. The main diagnostic hypotheses were dentigerous cyst and odontogenic keratocyst. Surgical management consisted of enucleation of the lesion with extraction of the right maxillary third molar. Histopathological examination demonstrated a parakeratinized stratified squamous epithelial lining with palisaded basal cells, confirming the diagnosis of odontogenic keratocyst. Six-month follow-up showed satisfactory healing, and long-term surveillance was recommended.
