Resumo: Proliferative verrucous leukoplakia (PVL) is a high-risk potentially malignant disorder characterized by multifocality, persistence, and unpredictable evolution. A 70-year-old female former smoker has been monitored since 2015 for non-scrapable white plaques affecting the tongue, interdental papillae, and alveolar ridges. Initial histopathologic assessment revealed leukoplakia without dysplasia. Between 2016 and 2018, recurrent lesions required multiple surgical excisions. In 2019, epithelial dysplasia developed and progressed to severe dysplasia in 2020, supporting the diagnosis of PVL based on cumulative clinical and histopathologic findings. In 2025, incisional biopsies from three distinct sites demonstrated acanthosis and hyperkeratosis with mild dysplasia, reflecting fluctuation in epithelial atypia while maintaining a multifocal pattern. The patient remains under strict surveillance, with serial biopsies performed during follow-up and no evidence of malignant transformation to date.
