Resumo: Syphilis is a systemic infectious disease that may present with diverse oral manifestations, particularly in its secondary stage. A 46-year-old female patient, smoker and hypertensive, presented with multiple erythematous and ulcerated lesions of unknown duration involving the buccal and labial mucosa, dorsum, ventral surface and lateral borders of the tongue, and floor of the mouth. The multifocal distribution suggested a systemic infectious condition. Non-treponemal and treponemal serologic tests confirmed secondary syphilis. The patient was referred for medical management and treated with penicillin therapy, resulting in significant regression of the lesions. She remains under clinical and serologic follow-up. This case underscores the importance of recognizing oral manifestations as potential indicators of systemic disease.
