Resumo: Odontogenic myxoma (OM) is an odontogenic tumor of ectomesenchymal origin that rarely affects pediatric patients. Despite its benign nature, radical surgery is indicated to prevent recurrence. Therefore, this study reports the case of an 11-year-old female presenting with a one-year history of asymptomatic facial asymmetry. Intraoral examination revealed a tumoral lesion in the right posterior maxillary region, without ulceration or changes in mucosal color. Radiographic examination revealed a well-defined radiolucent lesion with extensive bone destruction in the right maxillary region, involving the maxillary sinus, orbital floor, and nasal cavity. Based on the clinical hypothesis of ameloblastoma and OM, an incisional biopsy was performed, and the specimen was sent for microscopic analysis. The histopathological diagnosis confirmed OM, and the patient was referred for surgical treatment. Under general anesthesia, a partial maxillectomy was performed. The patient recovery was uneventful, and there were no signs of local recurrence at the latest follow-up.
