Resumo: Despite extensive investigation, no gold standard treatment exists for oral leukoplakia (OL), partly due to difficulties defining lesion margins and preventing recurrence. Laser diode treatment has emerged as a minimally invasive option, although intraoperative margin identification remains challenging. This study describes a case series of OL treated with a surgical protocol combining Lugol's iodine-guided demarcation with diode laser ablation.
Case series: Fifteen patients with OL were treated using a standardized protocol. Lugol staining identified glycogen-depleted epithelium and guided surgical margins. Lesions were removed using diode laser surgery: blue laser (450 nm) in nine cases and dual diode laser (810-980 nm) in six cases. The procedure involved superficial epithelial vaporization followed by photocoagulation. Healing occurred by second intention with minimal morbidity. Three recurrences were observed. No malignant transformation occurred during one-year follow-up.
The combination of Lugol-guided demarcation and diode laser ablation may represent a reproducible conservative surgical approach for selected cases.
