Resumo: A 54-year-old female patient, with no comorbidities, presented with severe transverse maxillary deficiency, bilateral posterior crossbite, and upper arch collapse, refractory to multiple orthodontic attempts. Clinical evaluation combined with cone-beam computed tomography evidenced significant skeletal maxillary constriction, incompatible with predictable correction by isolated dentoalveolar expansion. Surgically assisted rapid palatal expansion with a tissue-borne and tooth-borne Haas expander was indicated. Subtotal Le Fort I osteotomy with disjunction of the median palatine suture was performed. Progressive activation promoted an approximate transverse gain of 6 mm in the canine and molar regions, confirmed by comparative tomographic analysis. The postoperative period evolved without complications, with adequate new bone formation and transverse stability during follow-up, enabling the continuation of orthodontic treatment.
