Resumo: Objective: To propose a dysplasia-oriented clinical management protocol for actinic cheilitis based on current scientific evidence. Study Design: Narrative literature review performed through analysis of published studies addressing clinical features, histopathological grading of epithelial dysplasia, and therapeutic approaches for actinic cheilitis. Results: Histopathological grading of epithelial dysplasia is essential for risk stratification and therapeutic decision-making. Lesions with mild dysplasia may be managed conservatively with strict photoprotection and clinical monitoring. Moderate dysplasia may require topical therapies, photodynamic therapy, or minimally invasive procedures. Severe dysplasia requires complete surgical excision, including vermilionectomy when indicated, to ensure adequate control and reduce the risk of malignant transformation. Long-term follow-up is recommended in all cases. Conclusion: A treatment protocol guided by epithelial dysplasia grading may support clinical decision-making and contribute to improved management of actinic cheilitis.
