Resumo: Central vascular malformations of the mandible represent potentially life-threatening conditions due to the risk of severe hemorrhage during dental procedures. A 52-year-old female presented with a 10-year history of progressive, asymptomatic purplish swelling. Extraoral examination revealed violaceous enlargement of the right lower lip. Intraoral examination showed diffuse purplish swelling involving the floor of the mouth and tongue. Panoramic radiography demonstrated a well-defined radiolucent lesion in the right mandibular body between the canine and premolar. Computed tomography revealed lingual cortical perforation. All associated teeth were vital. Aspiration prior to biopsy yielded blood, confirming the diagnosis of central vascular malformation. The patient underwent surgical management and remains under follow-up, with satisfactory clinical evolution and no reported complications.
