Resumo: Background: Khat is a psychoactive stimulant drug prominent in Eastern Africa and the Arabian Peninsula, with increasing global consumption. Although adverse systemic and oral effects are documented, it's role in the pathogenesis of oral potentially malignant lesions and oral cancer remains unclear. This study employed liquid-based cytology in screening oral mucosal changes among adult male khat chewers in Kenya.

Materials and Methods: A cross-sectional descriptive study was conducted between 2018 and 2019. Data on sociodemographics, khat chewing habits, associated habits, oral examination findings, cytopathology, and HPV DNA polymerase chain reaction testing were collected and analysed.

Results: Forty-nine adult males (21 to 64 years; mean age 32.89) participated. Approximately half reported cigarette smoking (55%, n=27) and alcohol use (49%, n=24). Most showed non-neoplastic cytology (91.8%, n=45) with atypical reactive changes (6.1%, n=2). HPV DNA PCR was negative in 91.8% (n=45), with 8.2% (n=2) non-diagnostic.

Conclusion: Most participants exhibited non-neoplastic cytology. Tobacco and alcohol use were not significantly associated with oral mucosal changes (Fisher's exact test p-value > 0.05).

Recommendation: Further research is recommended to explore oral effects of Khat, including concurrent tobacco and alcohol and high risk HPV infection. Liquid-based cytology shows promise in population screening and as an ancillary test.
