Resumo: A 75-year-old male smoker was referred to the Stomatology Department due to a sublingual lesion present for 18 months. He had no systemic conditions or extraoral signs. Intraoral examination revealed a painless, firm submucosal nodule covered by intact mucosa, located in the left floor of the mouth, measuring approximately 6.0 × 3.0 cm. Initial diagnostic hypotheses included benign mesenchymal or salivary gland neoplasms. An incisional biopsy was performed, and histopathological analysis confirmed marginal zone lymphoma. The patient was referred to the Onco-Hematology Department for treatment. This case is notable due to the rare presentation of marginal zone lymphoma primarily in the oral cavity. It underscores the importance of early diagnosis, even for lesions with a benign appearance and slow growth, to avoid delays that could compromise prognosis. Marginal zone lymphoma is an indolent subtype of non-Hodgkin lymphoma and occurs infrequently in the oral region, making this report particularly relevant.
