Resumo: Low-grade central osteosarcoma (LGCOS) is a malignant bone forming neoplasm which originates within the medullary cavity. Accounts for about 1-2% of all osteosarcomas, presents equal gender distribution, preferentially affecting young adults. Most LGCOS cases affect the metaphysis of long bones, while jaw bones and ribs are rarely involved. By microscopy, fibroblastic-like tumor cells with mild nuclear atypia surrounding well-formed neoplastic bony trabeculae are usually found. However, morphological variants, mimicking fibrous dysplasia, desmoplastic fibroma, osteochodroma and chondromyxoid fibroma, create diagnostic pitfalls and challenging correct diagnosis. A 61-year-old female patient presented with an extensive osteolytic lesion on the right side of the mandible, which, after incisional biopsy, suggested odontogenic myxoma. However, a thorough evaluation revealed cellular atypia and focal areas of new bone formation and osteoid deposits. The diagnosis of LGCO, myxoid variant, was established, with the surgical specimen showing similar findings. To our knowledge, this variant has not been previously reported.
