Título: The Hidden Anatomy of the Mandible: Panoramic X CBCT analysis

Resumo: This study analyzed 1,027 imaging examinations, including panoramic radiographs and cone-beam computed tomography scans, to identify anatomical variations in the course of the mandibular canal, such as the anterior loop, incisive canal, and multiple mental foramina. Among the 100 hemimandibles assessed using panoramic radiographs, 85 percent exhibited at least one anatomical variation of the mandibular canal. In cone-beam computed tomography scans, this prevalence reached 90 percent. A higher prevalence of these variations was observed in patients under 42 years of age. The anterior loop was most frequently identified on panoramic radiographs, whereas the incisive canal was more often detected using cone-beam computed tomography. Overall, cone-beam computed tomography enabled the visualization of a greater number and diversity of anatomical variations. These findings underscore the clinical relevance of recognizing such variations to prevent surgical and anesthetic complications, misdiagnosis of bone pathologies, and irreversible damage. Despite the widespread use of panoramic radiography in treatment planning, its limitations must be acknowledged. Cone-beam computed tomography demonstrated superior diagnostic accuracy and should be considered an essential tool for comprehensive anatomical assessment and safe surgical planning in the mandibular region.
