Resumo: The aim of this study was to evaluate clinicopathological features of odontogenic keratocysts (OKCs) diagnosed at a single institution in Brazil. Between 2000 and 2023, clinical data were collected from the anatomopathological request forms, and each case was microscopically revised to confirm the diagnosis and register the detailed microscopic features. Data from 65 OKCs were collected, showing a slight female predilection and most frequently affecting individuals between the third and fifth decades of life. The posterior mandible was the most affected site. Microscopically, all exhibited hyperchromatic and palisaded basal epithelial cells, a flat interface between the epithelium and connective tissue, a corrugated parakeratin layer, and detachment of the epithelial lining from the fibrous capsule. Other observed features included the presence of microcysts and satellite cysts (18%), squamous islands (43%), dystrophic calcification (13%) in the connective tissue, as well as papillary surface projections (8%), budding (7%), and reversed polarity of basal epithelial cells (12%). When inflammatory infiltrate was present (68%), epithelial hyperplasia was commonly observed (39%). The presence of uncommon microscopic features may represent a challenge for diagnosis, especially the presence of inflammation, squamous islands and reversed polarity of basal epithelial cells.
