Resumo: A 41-year-old woman presented to an oral diagnostic center with a two-month history of mouth burning and an erythematous palate. Intraoral examination revealed erythematous macules and scrapable white plaques on the hard palate and tongue, associated with a provisional upper partial denture. A clinical diagnosis of multifocal oral candidiasis was made. Initial management included topical antifungals (nystatin and miconazole nitrate) resulting in mild symptom improvement without lesion resolution. Persistent symptoms and systemic signs (excessive hair loss) prompted hematological tests (CBC, glucose, iron, B12, HIV serology). Positive HIV serology led to referral to an infectious disease hospital, where antiretroviral therapy was initiated, and topical treatments were discontinued. Four-month follow-up showed significant clinical improvement, though palatal lesions related to the denture persisted, requiring ongoing oral rehabilitation. Holistic and systemic evaluation were critical for early diagnosis and disease control, underscoring the dentists role as a health professional in identifying systemic conditions.
