Resumo: Plasmacytomas are uncommon solitary malignant lesions that predominantly affect long bones; involvement of the jawbones is rare, accounting for approximately 4% of reported cases. A 41-year-old female patient, a non-smoker and non-drinker, presented with painful swelling in the left retromolar area. Clinical examination revealed facial asymmetry, limited mouth opening, and mucosal ulceration adjacent to tooth 38. Panoramic radiography showed an extensive radiolucent osteolytic lesion involving the left mandibular ramus. An incisional biopsy revealed a diffuse proliferation of plasmacytoid cells with varying degrees of pleomorphism. Immunohistochemical analysis was positive for CD138, plasma cell marker, lambda, and Ki-67 (>25%). The patient was referred for medical evaluation and further testing, confirming the plasmacytoma diagnosis. The patient was referred for treatment planning, which will involve surgical intervention and may include radiotherapy and adjuvant chemotherapy. Early diagnosis of jaw plasmacytoma is essential due to its aggressive behavior and potential for systemic progression.
