Resumo: Ameloblastomas are typically benign odontogenic tumors, but in rare cases, they may exhibit malignant behavior and metastasize. Common metastatic sites include the lungs and lymph nodes, depending on the route of dissemination. We report the case of a 66-year-old female with metastatic ameloblastoma involving the submandibular gland. Her history included multiple recurrences following initial surgery in 1972. More than a decade after the last recurrence, she presented with swelling in the submandibular region. Clinical and radiological evaluations led to the decision to perform a total excision of the right submandibular gland, along with a biopsy of a regional lymph node, which was free of neoplastic involvement. The patient remains disease-free after seven years of follow-up. This case highlights that metastasis in ameloblastoma may result not only from the tumor's biological behavior but also from factors related to surgical manipulation, which could contribute to recurrence and spread to atypical anatomical locations.
